What is PACE?

Program of All‑Inclusive Care for the Elderly (PACE) is a comprehensive health program for adults 55+ who want to keep living safely at home and in their community.

  • Provides medical care, medications, therapies, transportation, and social support

  • Coordinates all care through one team

  • Helps prevent unnecessary nursing home placement

  • Covered by Medicare & Medi‑Cal for those who qualify

Who Qualifies?

To enroll in PACE, a person must:

  • Be 55 or older

  • Live in the PACE service area

  • Meet the state’s criteria for nursing‑home level of care

  • Be able to live safely at home with PACE support

 

A PACE assessment helps determine if the program is the right fit.
